Personalised Care

Nursing Support Built Around You

Every participant has different needs, preferences and goals. That is why First Guardian Health Group provides personalised nursing support rather than taking a one-size-fits-all approach.

We work closely with participants, families, nominees, support coordinators and plan managers to understand the support required and deliver care with dignity, respect and clinical attention.

Whether you require ongoing community nursing, assistance with a complex health condition or short-term support following a hospital stay, our team is here to help you understand the available options.

Funding Eligibility

Using Self-Managed or Plan-Managed NDIS Funding

Self-managed and plan-managed participants may be able to use their NDIS funding for suitable nursing services where the support aligns with their plan, goals and available funding.

Funding arrangements can vary between participants. Before services begin, participants should confirm eligibility with their plan manager, support coordinator or NDIS contact.

Plan managers may contact First Guardian Health Group to request service information or a quotation.
